Here’s what we know about Isaiah’s true motives, Cam’s final choice, and how the film reveals who truly becomes the GOAT.

What happens in Him ending?

In the final act of Him, Isaiah forces Cam into a fight to the death to decide who inherits the title of the GOAT.

Cam kills Isaiah and becomes the new “greatest.” Isaiah dies smiling, satisfied that his legacy continues. After the fight, Cam reaches a football field where the San Antonio Saviors’ owners, his agent Tom, and Isaiah’s wife Elsie wait for him. They push him to sign a contract that cements his role as the next GOAT.

The film then exposes the full scope of the system that traps Cam. The wealthy, all-white owners use demonic imagery to show how they see Cam only as a commodity. They want to enrich themselves, just as they exploited Isaiah. This moment connects to Isaiah’s earlier transfusion of blood into Cam, which symbolized the passing of greatness. It also links to Cam’s father, who made a deal years earlier with the same cabal to secure his son’s rise.

Do Isaiah & Cam live or die at the end of Him?

Isaiah dies at Cam’s hands in the climactic fight. His desire to remain the GOAT is overridden by the brutal duel he demanded. Cam survives the confrontation and faces pressure to submit to the owners’ control, just as Isaiah once did. Instead, he resists their plan and sparks a violent rebellion. He kills the trainer and the team owners in the process.

This outcome emphasizes Cam’s refusal to become a puppet for the system that exploited Isaiah and countless other athletes. Unlike Isaiah, who sacrificed his identity to chase eternal greatness, Cam kills his way to freedom. He rejects the life of control offered to him. By doing so, he escapes the cycle and redefines greatness on his own terms. The story crowns him as the true GOAT.
